Lens Recommendation Menu

     The Patient Lifestyle Questionnaire and Doctor's Lens Recommendations Menu are printed on both sides of a single sheet of paper, along with a description of our guarantees and an optical shop discount coupon. This four-part document travels with the patient throughout the visit.

     In addition to being an efficient means of transferring information between team members, the lifestyle questionnaire engages the patient in thinking about the importance and use of eyewear in his life.

     The technician gives the sheet to the patient after the initial evaluation is completed. While waiting to see the doctor, the patient fills out the lifestyle questionnaire. Patients take the questionnaire very seriously and welcome the chance to discuss it with their doctor. The patient is given the opportunity to express his needs, and physicians, myself included, respond to them with sound recommendations. I use the lens menu as a checklist and briefly explain any feature that I recommend. When I bring the patient to the optician, I hand over the sheet with my, recommendations.
