Examples of What to Ask and Why

Are you bothered by glare from any of the
following: night driving, sunshine, fluorescent
lights, computer screens?
- WHY: Helps pinpoint whether the patient
will benefit from an antireflective or other
lens coating, variable tint optics, sunglasses,
etc.

Does your work entail unusual visual demands
due to: distance, position, lighting, other?
What are your hobbies: cards, woodworking,
bingo, fishing, golf, gardening, other?

- WHY: The more accurate a picture the doctor
has of her patients' visual needs, the more
precise and useful the recommendation will
be. The answers to these lifestyle questions
can point the doctor toward a recommendation
of computer glasses (for work at intermediate
distances), safety glasses (woodworking),
polarized lenses (fishing), etc.

If "no," what improvements would
you like?

- WHY: Pinpoints needs that aren't being
met. Allows patients to voice their concerns
